Both parents are required to complete and sign this form to register their child.
If parents would prefer not to complete this form together, we can accept two separate forms.
To be able to register your child, we require permission from all parents/carers with parental responsibility.

Please sign to agree that both parents /Carers can have access to your child's records and all information on both parts of the Registration Form are correct.

I agree to abide by the terms and conditions and policies and procedures of Tigers Day Nurseries which I/We have read and fully understand.
